ADR 0047 decides that a decided record needs assent from a contributor who is not the author of the change. With one contributor, adr check reports NOT APPLICABLE, and madtea's plugin hook asks the person for the token instead.

Children, in the order the work can run:

#655 find the author of a status change as the forge PR author.
#656 count contributors by access to the repo.
#661 enforce sign-off on decided records. Needs #655 and #656.
#671 a decided record with no decision issue fails, except the init record. Needs #661.
#672 later status changes and edits need sign-off. Needs #661.
#662 the plugin hook refuses an agent commit that decides or edits a record without the person's token. Needs #656.

Done when every child is closed. This work ships in a later breaking release. It was deferred out of v0.20.0 to keep that release focused. ADR 0047 is accepted; its enforcement ships then.
