Summary

adr check's decision-issues check now reads a linked issue on the HOST in its
URL, not the record's own forge, and never reads a different issue of the same
owner/repo/number on the record's forge. When madtea has no login for that host,
the check fails, names the link, and says to log in to that forge or use the
skip flag.

A decision issue named by a full URL is read on the host in the URL, with
that host's credentials, never the record's own forge. So an owner/repo#n
that also exists on the own forge is not read in its place. A URL on a
forge madtea has no login for fails, names the link, and says to log in to
that forge or pass --skip-remote-issues. A bare #n or owner/repo#n still
reads the checkout's own forge.

A forge-tagged test reads a real forge issue by its URL; the unit tests
cover the no-login failure and that the own forge is never read for a
cross-forge link.
