Summary

adr check, adr new, and adr init now refuse a docs/adr tree and name the help
topic with the steps to move the records to docs/decisions; the no-tree error no
longer mentions docs/adr. The legacy code (checkLegacy, newLegacy,
renderTemplate, the legacy layout branch) and its tests are removed. New
accepted record ADR 0054 records the removal; ADR 0034's legacy-layout clause is
edited in place to point to it.

adr check, new, init, and rebuild refuse a docs/adr tree and say to move
each record into docs/decisions; the refusal names the adr-legacy help
topic, which lists the move steps. docs/decisions (MADR 4.0.0) is the only
layout. The no-tree error no longer mentions docs/adr. The legacy check,
the legacy new, the house template renderer, and the legacy branch of the
layout detection are gone, with their tests.

Record this in ADR 0054 (accepted) and edit ADR 0034 in place to point at
it. Update the adr help, the reference docs, and docs/COMPARISON.md.
