ADR 0025: Status → Accepted; amend §3 with the explicit tie-break (unanimous scan > global; MIXED defers to an explicitly-set global; mixed without global fails closed; empty scan falls through); Tracking → this epic.
docs/COMPARISON.md auth row: terse mention "login origin-derived; --global deprecated (ADR 0025)" (row cap enforced by TestCompatNotesStayTerse).
CHANGELOG (if present): behavior change — origin now outranks a global madtea.url for repos with a remote; per-repo override for API-host≠git-host installs; host-keyed credentials with migrate-on-first-use; login --global deprecated.
Regenerate reference docs (go run ./cmd/madtea-dev docs generate); stale-doc drift is gate-enforced.
Also fix the stale doc comment at internal/config/config.go:301 (source list missing "origin remote").

Depends on: all code issues (docs regenerate last).

Acceptance: go test ./internal/docs/ ./internal/parity/ green.
